You can now download and print (if you have a double sided printer – even better) the Risk Assessment form prior to your visit to the clinic – this will give you more time to find some of the information which may be in your own private files or perhaps on your GP's surgery records.
Just complete the first page – the second is for the use of the clinic in recording any treatment you may have – you will need to sign the form when you visit the clinic (not before) to ensure all the information is accurate.
Important: Please remember to bring the completed form with you to your appointment. Only sign the form in the presence of clinic staff.
